If you are a referring physician or
clinical staff member and would like to request access to the Minnesota
Diagnostic Center imaging portal, both
a user
level and organization level Business Associate Agreement are required. Please
click the "Business Associate Agreement" button below to complete the
user level agreement. Once completed, you will be directed to our request form. An organization level Business
Associate Agreement will be emailed to your company’s compliance officer
following completion of the request form.
